If you have not seen Justin previously, please contact us to schedule your first appointment. We accept most major insurance plans. Please call, email, or use the “Contact Us” submission form to submit your insurance information to verify coverage before booking. Be sure to include the following:
The name of your insurer/plan (e.g. Blue Shield, Western Health Advantage, etc.)
Your full name, as it appears on your insurance card
Your date of birth
Your medical record/ID number
Group number
All new patients must complete our intake form prior to your first visit, accessible through Jane. We look forward to meeting you!
Returning Patients:
Welcome back! If you are a returning patient to Antidote, or have seen Justin prior to the opening of Antidote, you are welcome to use our online booking system through Jane. If this is your first visit to Antidote, please complete the intake form and include your insurance information (if applicable) prior to your appointment, as we do not have access to any information you may have provided to Justin’s former clinic. We’re so happy to see you again!
